Quote Requirements
Following is the information IBA requires to provide a benefit plan quote.
1. DETAILED CENSUS (age,sex,marital status, zip codes, salaries)
2. CLAIMS EXPERIENCE – BROKEN OUT MONTHLY WITH ENROLLMENT (at least two years of recent claims history)
3. LARGE CLAIMANTS – $15,000 OR MORE IN PAID CLAIMS or POTENTIAL CLAIMS (diagnosis, claim amounts, current status of member)
4. COPY OF THE LAST CONTRACT / APPLICATION COMPLETED FOR STOP LOSS
5. COPY OF LAST BILL
6. BENEFIT BOOKLET(S) FOR ALL COVERAGES
